  • Athletic Consent Form EL3
  • Athletic Consent Addendum EL3A

The FHSAA Athletic forms MUST be completed and turned into the Front Office prior to any athlete participation.  Sports physicals for the 2022-2023 calendar year must be dated June 1, 2022 or later.
